New Clients Forms

Welcome and thank you for making an appointment with Preferred EAP. Please review and complete the following forms before your first appointment. Otherwise, time during the initial intake assessment will be spent completing this paperwork. Complete a full set of forms for each person to be seen in session.

• Communication Consent Form
• Consent Form (for children or dependents unable to sign for themselves)

Services provided by Preferred EAP are free of charge to eligible participants. However, if you fail to keep an appointment or cancel an appointment with less than 24 hours notice, that session may be deducted from the sessions for which you are eligible. If you need to cancel an appointment with us, please call 610-433-8550 well in advance.
